Derbyshire Home from Hospital Service
Who do we help?
The Derbyshire Home from Hospital service assists vulnerable adults of any age to come home after a spell in hospital. It also assists those who are living at home but are at risk of being admitted to hospital.
The help of a Home from Hospital Volunteer can be invaluable in settling people back into their own homes by supporting with small tasks such as shopping, transport to medical appointments or just being a listening ear. This is a short term service for up to six weeks to help prevent hospital re-admissions and to help people get home sooner.
Volunteers can offer support on a weekly, fortnightly or ad hoc basis, depending on what help someone needs and volunteer availability to support them.
How much does it cost?
The service is provided under tender by Derbyshire County Council and delivered in the North Dales and High Peak by Connex Community Support. Volunteer support is offered for a six week period, free of charge.
How do I get referred to the service?
The service takes referrals from anyone so someone can self-refer to the service or referrals can be taken from a friend, family member or nurse for example. In the first instance you should call 01283 817417, Monday to Friday between 9am and 5pm. Ths is a central number for all Home from Hospital services across Derbyshire. Your referral will then be passed to the right service for your area.
We like to meet people before they are discharged from hospital but people can access the service up to six weeks after they have returned from hospital. After six weeks, if support is still needed, you may wish to consider contacting our Befriending service or our Transport service, depending on your needs.
