Home from Hospital Coordinator

Duration:Initially fixed until 31 March 2027 (extension subject to extension of service contract)
Contracted Hours:21.5 hours/week (worked flexibly)
Salary:£ 30,925 FTE pro rata (£17,970 actual)

Based At:Connex Community Support office in Buxton with frequent travel across and occasionally outside the area.

Purpose:

Providing holistic support that enables individuals to be discharged from hospital feeling supported, confident, and empowered. This includes identifying and coordinating the practical, emotional, and social support needed to reduce the risk of hospital readmission or future admissions. All support is delivered within a strengths‑based approach that promotes wellbeing, independence, confidence, and overall quality of life.

Duties & Responsibilities:

  • To ensure the day-to-day delivery of the service in line with the contractual and organisational requirements.
  • Be the first point of contact for client referrals, making sure
    • they are processed within the contractually defined timescales
    • clients’ needs are sensitively and correctly assessed
    • the appropriate support for the client is identified and made available, either directly through the Coordinator, Connex volunteers, other Connex services, an external organisation or by signposting information
    • that where necessary, safeguarding or other concerns are escalated accordingly to ensure clients receive the support needed.
  • Work with staff from the statutory services to agree the contribution to be made by the service, liaising with the relevant statutory and voluntary agencies and the clients themselves. Where relevant, meeting with Discharge Planning Teams and other providers to resolve issues.
  • Provide low-level support for clients for up to 6 weeks, either directly and through home visits or by overseeing the provision of support through Connex volunteers; always ensuring support is flexible and responsive to any additional or changing needs.
  • Work closely together with the Connex Home from Hospital Coordinator for the Derbyshire Dales to ensure best use of resources in responding to referrals and to cover each other’s area during annual leave or sickness.
  • Establish and maintain effective working relationships with other internal and external teams, including from the service’ lead contractor (South Derbyshire CVS), Connex’ Patient Discharge Transport, Befriending and other Connex services, Hospital Discharge teams, Social Prescribers, Social Workers, Care Coordinators as well as relatives of clients.
  • Recruit, induct and supervise volunteers to help with the delivery of the service and ensure their ongoing development through appropriate support and training
  • Promote the service and its volunteering opportunities proactively and effectively to ensure performance targets can be met and using all available means, including printed and electronic publications, networking and attendance of meetings or other opportunities to present the service.
  • Collate all monitoring data required to demonstrate the outputs, outcomes and quality of the service and prepare the relevant reports for internal and external stakeholders.
  • Ensure that all data is processed and recorded in line with agreed processes and legislative requirements such as GDPR.
  • Undertake and update risk assessments for the service and its clients, taking the appropriate action with regards to any identified risks.
  • Assist in the service being delivered within the defined budget and ensure that all expenditure is correctly recorded.
  • Comply with all Connex Community Support’ policies, procedures and other relevant statutory requirements as well as with relevant procedures required by the lead contractor
  • Carry out any other duties as may be required from time to time.

Person Specification:

Essentials

  • Desire to provide support and help people, some of whom may be in difficult circumstances.
  • Patience and empathy for the needs and expectations of clients and referring agencies.
  • Very good listening, verbal and written communication skills.
  • Strong interpersonal skills and ability to build effective working relationships with people from a variety of personal and professional backgrounds.
  • Experience of working in a health or social care setting that is supporting people
  • Ability to undertake needs assessments and taking the right actions for implementing an individual’s practical, emotional and social support.
  • Ability to identify, manage and appropriately escalate safeguarding concerns.
  • Ability to work on own initiative and as part of a team.
  • Ability to enthuse people to volunteer for the service and to provide the effective training and support.
  • Good organisational skills and ability to handle conflicting work pressures.
  • Confident and self-sufficient IT user, particularly Microsoft Word, CRM, Excel, Outlook and Teams including for monitoring and reporting purposes.
  • Non-judgemental disposition and commitment to principles of confidentiality and safeguarding.
  • Ability and willingness to regularly travel across the work area, including rural and remote communities, and to attend external meetings with associated organisations, using your own reliable transport. Desirables:

  • Experience of working in service that seeks the reablement of people following a hospital stay or other health related changes in their lives.
  • Experience of working in an environment where multiple agencies may be involved in supporting a person at home.
  • Experience of working with volunteers, including their recruitment, training and support.

We are a small team and therefore expect all our staff to be versatile in their approach to the required tasks.

Disclosure and Barring Service Certificate:

Any job offer is made subject to the satisfactory completion of all required pre‑employment checks. This post will require a certificate of enhanced disclosure issued by the Disclosure and Barring Service which we will apply for on a candidates behalf.

If the DBS check is outstanding at the proposed start date, the individual must not undertake any regulated activity until the DBS has been received and approved. In such circumstances, the individual may either commence work in a non‑regulated role or have their start date delayed until the DBS process is complete.
